The New Foscote Hospital works with Definition Health, to ensure pre-operative assessments are accurate and convenient. You complete your part of the assessment online, using the secure LifeBox app, via a smartphone, laptop, or desktop.
Admission and procedural information can be accessed in the same place, along with related medical images, or helpful videos. All without needing to travel to a hospital appointment.
Our hospital has equal access, ensuring any concerns, or individual needs are identified early, or access arranged to previous records. Along with saving time, the LifeBox system contributes to ensuring you receive personalised care.
Being Part Of LifeBox
Once you are listed for a procedure, we will create a LifeBox account for you, using the name, mobile phone number and email address you provide. An email and/or text will confirm set up and provide a link to your account.
A straightforward, two step authentication process ensures good security. You choose a password and enter a code sent to you by text, then just hit sign up.
This gives instant access to your documents and a questionnaire you need to complete. Completion will take 30 to 40 minutes but everything is saved as details are entered, you can take a break and come back at any time.
As full a medical history as you can provide is needed, at least for the last 5 years. Information on past and current medication should be added, along with your height and weight, which can be estimated if you are not sure.

We ask that you complete the questionnaire within 7 days of receiving the link. This is an uncomplicated process but if you hit a problem, our team will help.

Should you not use online facilities, or have an email address, we will set up an account for you. A face to face appointment can then be organised.

Should you hit a stumbling block:
Contact us at any stage and one of our team will call you. They can then guide you through outstanding points to finalise the questionnaire.

Options are built in to help if you forget your password. Should you wish to talk with LifeBox about anything else, there is a chat facility in the app.
